IF YOU HAVE HAVE DONATED MORE THAN $15 AND WANT TO USE MORE THAN 1 RAFFLE TICKET IN THIS RAFFLE, YOU WILL HAVE TO SUBMIT THE FORM AGAIN.  EACH TIME THE FORM IS SUBMITTED, YOU WILL BE ENTERED 1 TIME INTO THE RAFFLE.

Please note there are a few prizes that are for Los Angeles residents only (because they are in person services, not because we don’t love you). The prizes that are LA only are marked as such.
